Appendix D: Website Supplement For Users of the 3sHealth Website

1. Collection of Personal Information

3sHealth may collect personal information (PI) from website users where the user chooses to provide that information via 3sHealth's website. 3sHealth has implemented safeguards to protect the information collected via its website. However, the transmission of information via the internet is inherently insecure. As such, please do not submit sensitive information via the 3sHealth website.

 2. Collection of Anonymous Data

Visitors to 3sHealth’s website should also be aware that anonymous technical information may be collected by 3sHealth as a result of a visit to 3sHealth’s website. For example, this information may include the visitor’s IP address, browser type, operating system, domain name, access times, and referring website addresses. 3sHealth uses this anonymous technical information for purposes such as diagnosing problems with 3sHealth’s servers, improving the operation and content of 3sHealth’s website, and compiling aggregate and statistical information.

3sHealth will not attempt to link or match such anonymous technical information with any personally-identifiable information unless 3sHealth has an individual’s consent, 3sHealth (or its service providers) have detected or reasonably suspect any unlawful use of 3sHealth’s services or a security breach, or 3sHealth has a legal duty or right to do so.
